Alongside our usual services, we can offer
- A cancer care review appointment
- Emotional and physical health needs support and treatment
- Medical Certification ( sick notes ) for work
- Prescription fee exemption forms
- Prescriptions
- Looking after any other ongoing health conditions or new symptoms that arise
- Dressings / injectable therapies/ post operative care
- Signposting to other services
- Palliative care support e.g. assessments, referrals, DS1500 etc
- Advanced care planning
- Social prescriber team support
