The Integrated Care Team care for adults in their own homes, including care homes. We provide nursing, treatment and support in the home and local community for people with a range of health problems or conditions.
The team supports recovery, stabilising symptoms and helping people to manage their own conditions. Our patients may be housebound, elderly or frail and at risk of being admitted to hospital. Our service is for adults with a range of health issues, from illness or injury to longer terms conditions. We also support patients when they are discharged from hospital.
We work closely with GPs and other health and social care workers to give the care and support needed by each patient. Sometimes we will need to make a referral or consult with another specialist service, such as social services or a community mental health team.

You need to be registered with a GP at one of our surgeries to use this service. Contact your GP or health professional to discuss this service. They can make a referral to the Integrated Care Team if they feel the service would meet your needs.
