Charges will be made for the following private services:

  • Pre-employment Medicals 

  • Insurance Claims

  • Firearms Licence applications

  • Private Certificates

  • Holiday Vaccinations

  • Fitness to Drive Medicals

  • Private Health Insurance Forms

  • Seat Belt Exemption Certificates

  • To patients not entitled to NHS care

  • Copying Medical Records 

Some other services may be chargeable, you will be advised of this at the time of your request.  SAE must be supplied if forms are to be returned by post.  We take card payments and cash. 

Please note that this work is not part of our NHS contractual requirement and our doctors are not indemnified to complete such requests. You will also be aware that general practice is under unprecedented pressure and therefore it may not be possible to carry out your request, since this would detract from our ability to provide core contracted primary care services to our patients.

It is for this reason that we no longer provide fit to fly letters  / diving fitness medicals / sports medicals / charity event medicals

We are unable to make recommendations, however, you may be able to find a private service locally who can assist.

 

PIP Applications – Supporting Letters Policy
When you submit a Personal Independence Payment (PIP) claim, the Department of Work & Pensions (DWP) will usually contact your GP or healthcare professional to obtain medical evidence.

GPs are required to provide statements of incapacity to an officer of the DWP or a healthcare professional working for the Health Assessment Advisory Service on behalf of the DWP when requested.

Please see below for DWP advice on support for PIP claims. You will see that they do not recommend patients requesting letters for support from your GP for PIP claims. However, you are able to get all the information you require yourself from your medical record, which is available to you online via the NHS App. Alternatively, if you wish, a copy of your medical records, please contact the practice who will advise you accordingly. There is no charge for a copy of your records, however if you request a further copy within a twelve month period then a fee may be incurred.

Supporting information for Personal Independence Payment (Easy Read)

If the DWP or the Tribunal Service request further evidence from a GP in relation to an appeal, they will ask us for this directly and will be responsible for paying any fee to the doctor providing the report. We do appreciate that navigating the benefits system can be daunting. If you require any advice or support around this please ask to speak to one of our Social Prescribing team who should be able to assist you. Should you still wish to proceed with a request for a supporting letter please be aware that this is non-NHS work. All requests will therefore attract a charge.
